MHR Services

Before services begin, you will meet with your MHR team to develop a treatment plan called an Individualized Service/Recovery Plan (ISRP). As a key decision maker, you help set the goals for yourself or your child.

 

 

How often will I get services?

While you and the team are developing your plan, you will decide which services will help you the most and how often each service should be received. You may receive more than one service each week. Services may be provided in your home, at school, or in other community locations.

 

 

Where and when are services provided?  Will staff come to my home?
One really good thing about the MHR Program is that the services you need can be provided where they are needed rather than only at the MHR agency. You may meet with agency staff members in your home, at school or at other community locations.

What services are not included in the program?

Medicaid will pay the MHR agency of your choice to provide you with MHR services. However, Medicaid will not pay an MHR agency to provide the following services:
 

 

bullet Transportation
bullet Recreational outings
bullet Teaching job skills
bullet Tutoring

 

The agency you choose may decide to offer these services, but they are not required to do so and Medicaid will not reimburse them. If you need Medicaid transportation to and from the MHR agency you choose, you may use Medicaid transportation.

Once you have selected an MHR agency, your Community Support Worker should be able to help you to set up transportation on a regular basis and help you find other programs that can provide tutoring, job training, or information about recreational activities near your home.
