DHH has a standard set of forms providers must use for the
provision of MHR services which include the Initial Assessment form, the
Reassessment form and the Individualized Service/Recovery Plan (ISRP)
format. Requests for authorization must be submitted on these required
forms and they may not be modified in any way. Failure to submit the
correct form will delay the authorization process.
DHH communicates with providers in a variety of ways including this
website, letters from DHH, and provider WRAP Sessions. This page of the
site includes new and archived letters from OMH and/or Medicaid and WRAP
Session notes, which are posted for providers who are not able to attend
quarterly Provider WRAP sessions.
