Consumer Rights and Responsibilities

Any adult with a serious mental illness or child with an emotional/behavioral disorder may be eligible for Mental Health Rehabilitation Services. These services are provided in the community, home or school to assist adults and children to remain in the community. Anyone who thinks they may need Mental Health Rehabilitation Services may contact any of the agencies on the "Freedom of Choice" list to obtain these services. Some of the services you might expect to receive are counseling, medication management, psychosocial skills training and assistance getting any services you might need.

 

Choose the Parish you live in, from the list below, to view the MHR Provider "Freedom of Choice" List for your area. These files are updated regularly and are the most current Choice lists available.

Once you chose a provider you may call them to start the process to receive services. The agency will complete an assessment to determine your strengths and needs. You and the agency staff will then write a plan for your care that meets your needs. Services will then be provided based on that plan. If you have any concerns or questions about those services you may call 1-800-558-4618 to talk about your concerns. This is a toll free number to the Office of Mental Health (OMH). OMH is responsible for insuring the quality of services for people who receive the service.

 

NOTE:

Every person who receives Medicaid Mental Health Rehabilitation Services has the right to pick a provider of their choice. You may wish to review the information at the following website to insure you chose the best agency to meet your needs.
